The article aims to understand the actions in Primary Health Care in Campos dos Goytacazes/RJ, of relevance in the production of oil and gas, and characterized by territorial heterogeneity - highlighting the locations of Murundu and Conceição do Imbé, predominantly rural and far from the municipal headquarters. The methodology covers a bibliographical survey on the national policy of Primary Health Care and the reverberations that reflect on the reality of health actions at the local level. A deficiency in the “conversion” or in the application of national policies to the reality of the territories surveyed is perceived, lacking a conjunctural analysis between managers, professionals and communities, decentralizing the confrontation of issues related to access to services and their obstacles to resolution. The municipal health management model, following the principle of equity, must observe socio-spatial inequalities in guaranteeing access, in order to transform the conditions that increase disease rates, such as vulnerability.
